The Clinical Observation in Kawasaki Disease & the Comparison of the Five Consecutive Dose Therapy of lntravenous Gammaglobulin


Abstract
The 37 cases with Kawasaki disease who were diagnosed and treated at the department of Pediatrics, Pusan Il Sin Christian Hospital from January 1987 to October 1992, were investigated for clinical patterns and laboratory correlations.
@ES The results were as follows;
@EN 1) Most patients(94.6%) were under 4 years of the age and boys were prevalent than girls, as a ratio of 2.7:1.
2) The total duration of fever was 9¡¾3.27 days and the duration of fever after admission was 3.26¡¾2.23 days as a mean.
3) The incidence of principal symtoms were fever over 5 days(100%), erythematous fissure lip and strawberry tongue(97.3%), edema of hands and feet(86.5%), bilateral conjunctival injection(83.8%), skin eruption(81.1%), desquamation(70.3%),
cervical
lymphadenopathy(59.5%).
4) The laboratory findings observed were anemia(5.68%), leukocytosis(100%), thrombocytosis (73%), increased peak level of ESR989.2%), positive CRP(100%), increased SGOT(43.2%), increased SGPT(45.9%), hypoalbuminemia(40.5%), proteinuria(8.1%),
pyuria(64.9%).
5) The abnormalities of chest P-A were 67.6%, that of electrocardiography were 37.8%.
6) Coronary aneurysm and dilatation by echocardiography was found in 18.9% of examed cases, and the sites of coronary aneurysms were left coronary artery(71.4%), right coronary artery(14.3%) and involved coronary arteries were usually unilateral
(85.7%).
7) As coronary aneurysm developing groups compared with non-developing group by clinical and laboratory findings, the total duration of fever was longer significantly in coronary aneurysm developing groups (11.67¡¾4.84days vs 8.44¡¾2.64 days;
P<0.05).
8) The mean duration of fever after high-dose intravenous gamma globulin was 1.21¡¾2.05 days, the single dose therapy of intravenous gamma globulin was not superior statistically to the five consecutive dose therapy of intravenous gamma globulin
in the
duration of fever90.48¡¾2.53 days vs 1.35¡¾1.97 days).
